Clinical Director of Case Management/Social Services

We are proud to be a Magnet recognized organization, and are currently seeking a full-time Clinical Director of Case Management/Social Services to join our team.
Reporting to the Vice President and Chief Informatics Officer, the Clinical Director is responsible for the management of all aspects of Case Management, Utilization Review and Clinical Social Work. The Clinical Director will have a strong knowledge of compliance and regulatory standards governing health plans and patient care.
Requirements include a Bachelor’s degree in Nursing; Master’s degree preferred. A minimum of five years clinical/supervisory and hospital-based Case Management experience with extensive knowledge of payer mechanisms and proven critical thinking, analytical and strategic planning skills also required. The ideal candidate will have excellent verbal and written communication skills including knowledge of Meditech Information Systems and current IL RN licensure. Board Certification in Case Management is preferred.

Clinical Director of Nursing
Serving the residents of Chicago's north and northwest communities for nearly 120 years, Swedish Covenant, a Magnet designated hospital, offers fulfilling, long-term careers in a humanistic, technologically sophisticated, teaching environment.
We are currently seeking a Clinical Director of Nursing for our Rehab Unit, the Extended Care Unit, and a Med-Surg unit. These specialty units have strong Interdisciplinary Team models, which include the Clinical Director, a physician, PT, OT, other therapists, and Social Services. As an experienced nurse administrator, the Clinical Director is responsible for clinical excellence in these specialties, and will ensure well-planned organizational structures, inclusive decision-making, creative and supportive personnel policies, professional models of care that promote excellence in patient care services, collegial interdisciplinary relationships and professional growth and development for the Division of Nursing.
Requirements include a minimum of five years management experience within a Rehab, long-term care and/or hospital setting, current IL nursing licensure, and excellent verbal and written communication skills, including knowledge of Meditech Information Systems. Board Certification in a nursing specialty and a Master’s degree are preferred.
